What is HIV/AIDS Case Management?
It is a free voluntary program that links HIV+ individuals with health care, psychosocial and other services. A case manager will meet with interested individuals to:
- Assess the condition of the client’s health
- Identify the client’s needs for health care and supportive services
- Determine the client’s eligibility for various federal, state, and local programs
- Match the client’s identified needs to available resource and programs
- Enroll the client in the appropriate programs and services
- Instruct the client on how to access various programs and services
Who may be eligible for HIV/AIDS Case Management?
- HIV+
- Missouri resident living in Cole, Moniteau, Morgan, Miller, Camden, Maries, Osage, and Gasconade
What are the benefits and services provided by HIV/AIDS Case Management?
- Medications
- Physician services
- Laboratory services
- Housing & Utility assistance
- Mental Health services
- Dental services
- Transportation assistance which is medically related
- Health Insurance Premium Assistance including COBRA
These services are available only when there is no other way to pay for the services. They are provided based on program eligibility, assessed need and available funding.
How do I apply for HIV/AIDS Case Management?
- Call 573/636-2181 and a case manager will discuss eligibility criteria, collect personal data, and make an appointment.
- Submission of proof of HIV status, submission of personal identification documents, proof of residency, and proof of income are required upon enrollment.
