Why should I report a disease?
- The accurate identification and timely reporting of disease and environmental health conditions is an integral part of successful disease control that enables the health department to provide epidemiological follow up or disease intervention without delay.
- Also, reporting is required by state law ( Rule 19 CSR 20-20.020 and 19 CSR 20-20.080 ).
How do I know if I need to report a disease?
- If you are unsure about whether the condition meets mandatory reporting requirements please refer to the section entitled “Diseases and Conditions Reportable in Missouri” located on the Missouri Department of Health and Senior Services.
How do I report a condition or outbreak?
- Physicians and laboratories shall contact the Cole County Health Department by phone (573-636-2181) or fax (573-636-3851) to report disease conditions and outbreak situations or contact the Missouri Department of Health and Senior Services by phone at (573)-751-9071 or (800)-392-0271 or fax at (573)-751-6417.
- Physicians offices should please complete a Disease Case Report, commonly referred to as a CD-1 . Download and print a copy of a blank CD-1, and fax the completed CD-1, along with a copy of the lab work, to the Cole County Health Deptment at (573) 636-3851 as soon as possible.
