The information contained within this guide has been provided to the Cole County Health Department by each organization/company/agency listed. The Cole County Health Department assumes no responsibility for the validity of data or for errors or omissions in information. This guide has been designed to assist Cole county residents in accessing services which might help them in meeting their needs. It is intended as a guide to making first contact with the agencies and programs they provide.

Cole County is rich in the numerous available services offered to the public. Organizations that have chosen to participate in this guide are listed alphabetically. In this alphabetical listing, you will be able to read the detailed information that they have submitted. Please note that several organizations have numerous and various services they offer to the public.


2008 Community Health Assessment

In 2005, a comprehensive community assessment was undertaken in conjunction with strategic planning for the organization.  That report consisted of both primary and secondary data and incorporated the priorities of staff, leadership and the community.  

In 2008, the data was again reviewed to determine if new priorities were necessary. The content of this report contains both baseline and trended data for use by the many local organizations that collaborate with the Cole County Health Department on community improvement initiatives.  Topics addressed in this report, as in 2005, include social-economics of the community, leading causes of death emergency room visits, injuries, maternal and child health, environmental health, mental health, chronic disease and community protection. The data review indicated that the priorities established in 2005 remain important and the Health Department will maintain its focus on the issues of Healthy Lifestyles, injuries and Access to Health Services.

While every issue is not covered; nor every topic covered at length, data reflecting current trends is provided as well as progress toward implementing or sustaining interventions that address various issues. This data is being provided to ensure that progress toward improved health status is continually monitored and to offer community partners an opportunity to use this data in future decision-making regarding provision of programs and services.
