Onsite Sewage Disposal System Construction Permit Application

Thank you for contacting us concerning plans for your on-site sewage disposal system. As you may know, law has required the Missouri Department of Health to regulate the design, construction and installation of on-site sewage disposal systems. Cole County Health Department has chosen to regulate the on-site sewage systems locally.

Below are links containing forms and instructions to help you apply for a permit and to select an on-site sewage disposal system that will comply with the regulations. Construction of your on-site sewage disposal system SHALL NOT begin until Cole County Health Department has issued a permit. To expedite this process, please follow these steps:

  • Contact a registered/certified on-site sewage disposal system contractor. This contractor can best help you to construct your on-site sewage disposal system and IS REQUIRED. The services of a certified person to conduct a percolation test or a soil scientist to provide a soil morphology (the preferred method) ARE ALSO REQUIRED. The contractor can help you select a system to suit your needs, and will help you fill in the forms. You may also consult with the Cole County Health Department.
  • Fill in the "On-Site Sewage Disposal System Construction Permit Fee" form and submit it, along with the $100 fee, to the address on the form.
  • Use the "On-Site Sewage Permit Instructions and Check Off' form to ensure that you have gathered all of the required information. Then, submit the completed forms and all necessary drawings and plans to this office.
  • Upon receipt of the completed application packet by the Environmental Public Health Specialist (EPHS), a site visit must be conducted. Call the EPHS to schedule a site visit (installer must call a minimum of 2 working days before construction is to begin for site visit). If the results of the site visit and plan review is satisfactory and we have received the permit fee, we will issue the permit. Then, construction may begin.
